• Health information (e.g. medical history, diagnosis, treatment plan, medications, specialist reports, test results); 

• Other health information (e.g. family history, management by other health professionals);

We will only collect sensitive information about you that is reasonably necessary for us to perform our services, and only with your consent. This information enables our practitioners to properly assess, treat and be proactive in your health care needs.

How do we collect your information? 

We will collect and hold your personal and sensitive information in a fair and lawful manner, and not in an intrusive way.  

For example, we may collect the personal information through the following means: 

• Directly from you when you give us your details (e.g. in-person, over the phone, via email, client registration form, online enquiry form or an online booking form); 

• From an authorised person acting on your behalf;   

• From a third party where we are permitted by law (e.g. other health care professionals involved in your care, from your health insurer, from the My Health Record system etc.).

We collect, hold, use and disclose information, including your personal information, primarily for business purposes that are directly related to our business activities and when it is necessary and relevant to our relationship with you.  

For example, we may use the information we collect about you to:  

• To provide allied health services to you; 

• To be able to contact and communicate with you regarding appointments, services, sessions, events, updates; 

• To communicate with other health professionals involved in your care; 

• To process payments and manage billing, accounts and administrative services; 

• To conduct accreditation, quality assurance or internal audits; 

• To comply with applicable laws and regulations.

How do we disclose your information? 

We may disclose information about you to others outside of our practices as permitted or required under law. This will include situations where we disclose information about you in order:  

To comply with our legal obligations (e.g. mandatory reporting under legislations, responding to a court order or subpoena); 

• To consult with other health professionals involved in your health care; 

• To get test results from diagnostic and pathology services; 

• To claim on insurance; 

• To communicate with your health fund, with government and other regulatory bodies such as Medicare; 

• To help us manage our accounts and administrative services (e.g. billing or debt recovery, arrangements with health funds, pursuing unpaid accounts etc.);  

• To reduce or prevent a serious threat to a patient’s life, health or safety or a serious threat to public health or safety; 

• To help in locating a missing person; 

• To establish, exercise or defend an equitable claim through the My Health Record; 

• To prepare the defence of anticipated or existing legal proceedings; 

• To discharge notification obligations to liability insurers; 

• Other entities with your consent. 

 

Are you obliged to provide us personal information? 

 You are not required to provide us the information that we request, or to allow us to collect information from third parties.  However, where you choose not to provide us with the information we request, we may not be able to provide you with services that you have requested from us, and we may elect to terminate any arrangements we may have with you.  Importantly, if you provide either inaccurate or incomplete information to us you risk receiving services that may not be appropriate or suitable for your needs. 

 

Do we disclose personal information for marketing? 

We may use your personal information to offer you products and services that we believe may interest you. We may also disclose your personal information to external associates and service providers who assist us to market our products and services. 

If you do not want to receive marketing offers from us, please inform us.  Our contact details are included at the end of this policy.   

 

Storage and security of information 

We hold personal information as either physical records, electronically on our intranet system, in cloud storage.  We maintain security measures to protect your personal information from unauthorised access, misuse or disclosure.

We will destroy or deidentify information when it is no longer needed for a valid purpose.

• the access, disclosure or loss is likely to result in serious harm to you. 

• If you receive a statement of an eligible data breach from us, you should read and implement the recommendations about the steps you should take in response to the eligible data breach.

You can request access the personal information we hold about you by contacting us using the contact details at the bottom of this Privacy Policy. We may charge you a reasonable fee to cover our costs for providing access. We will disclose the amount of such costs to you prior to providing you with the information. There may be instances where we may not be able to provide you with access to all your personal information and, where this is the case, we will tell you why.  

We may also need to verify your identity when you request your personal information.  

If you think that any personal information, we hold about you is inaccurate, please contact us and we will take reasonable steps to ensure that it is corrected.
